This form is used to remit your CCCPLS Certification or Recertification Fee.
The certification fee is a one-time fee of $440 to have your application reviewed against the ISO Standard. There is no additional fee for the written exam if you sit the exam at the CAP-ACP Annual Meeting. If you select an educational institute or exam centre (from the approved list) you will be responsible for the sitting fee at that location.
There is a recertification fee of $120 Autopsy Technician (every 3 years) and $40 Annual Recertification of Biopsy Processing/Simple Dissection.
Certification and Recertification fees are non-refundable. Ensure you have all the correct documentation before you proceed. Approved applications may be audited for completeness and documentation verified. If it is determined that documents submitted do not meet the minimum requirement for the route, and if correct documentation cannot be provided (by the applicant), the decision may be revoked.
