Option 2 : Single day rates apply for single day registrations only.  Attendance at 2 or more days requires full conference registration (see above).  Accommodations must be arranged directly by the delegate. 
Auto-Transfers to MAINPORT EPORTFOLIO
Royal College Fellows and MOC Program participants who provided their identification number at the time of registration will have their MOC Section 1 credits automatically transferred to their MAINPORT holding area following the learning activity. To complete the entry participants will be required to record their learning outcomes.

Letter of Invitation
Individuals requiring an official letter of invitation in order to obtain a visa and authorization to attend the CCMG Conference should contact us by email at info@ccmg-ccgm.org . The invitation letter will be issued once the registration form and payment has been received by the CCMG. The letter does not constitute any financial commitment on the part of the Association.

Cancellation Policy:
Registration cancellation must be received in writing to the CCMG office on or before April 26, 2019. An administration charge of $50 will apply to all refunds. No refunds will be provided after April 27, 2019.
